$20.00 administrative fee for the 1st immunization

$10.00 for 2nd vaccine

$5 for additional same day immunizations

We accept Husky and most major

insurances. Please bring your insurance card.

• Parent/guardian must be with child at clinic.

• Bring child's vaccine records to clinic.
