The Stratford Health Department will be offering flu clinics this fall. All ages are welcome.

To ensure a safe and timely experience we are encouraging individuals and families to complete their vaccination forms ahead of time and bring a PHOTOCOPY of their license and insurance cards with them to the clinic.
The required forms can be found below. Please print and complete the consent form and bring all completed forms with you to the flu clinic. Don't have a printer? You can download and complete the forms on your computer and submit to Bernice Bova at bbova@townofstratford.com through our secure email portal here. Drive-through Flu Clinic Recommendations:
- Please wear loose clothing and/or short sleeves to help us administer the flu vaccine easier
- Masks are required
- Each individual in the car (adult or child) we need to have a consent form completed
- Please bring a PHOTOCOPY of each individuals insurance cards and photo ID (if applicable)
PLEASE NOTE: The FluMist Nasal Spray and the High Dose Vaccine WILL NOT be available at the Stratford Health Department flu clinics this year. We are offering Flublok in place of High Dose. What is Flublok? Learn more here.
Vaccines will be given on a first come, first served basis. All clinics are subject to vaccine availability and supply.
All others will be charged a fee payable by cash or check:
- Standard Flu Vaccine for all children 6 months to 59 months old and 5-18 years old who qualify for VFC: Free
- Standard Flu Vaccine for those 5 years and older: $35.00
- Flu Block Vaccine for those 18 years and older: $70.00
